About our project

The problem we solve: Many times patients do not catch leading indicators for chronic disease. Chronic conditions often require additional hospital visits and strain. T2 diabetes, and prediabetes can be caught early. If not caught, these diseases can lead to heart disease, stroke, kidney disease, eye problems, nerve damage and foot problems. U.S prevalence is 30.3 million individuals or 9.4% of the population. Globally diabetes impacts more than 420 million people. U.S. incidence is 1.5 million every year, and diabetes remains the 7th leading cause of death in the U.S. This chronic disease significantly impacts quality of life and total healthcare costs with U.S. costs in 2017 exceeding $325 billion. Therefore, this chronic disease, and its consequential circumstances, represents a significant and costly healthcare challenge positioned remarkably well for early preventative technology and continuous care to impact individual decision-making and lessen likelihood of disease incidence and hospitalization.

About our solution: Theraputix provides a new and connected service delivery model for disease prevention and care to address accessibility and cost, while maintaining effectiveness, safety and patient engagement. Through our platform, patients can employ secure, encrypted web and mobile apps to import existing electronic medical record and patient history into their profile. Dietary patterns and caloric intake can be input throughout the day, while activity data is passively recorded through a variety of smart watch and phone sensors ubiquitously available on consumer devices. This decreases barrier to entry and costs to improve accessibility. Utilizing this information, intelligent algorithms process and adaptively update metrics such as likelihood of disease onset, or life expectancy and complication likelihood if already diagnosed. Visualization tools engage users to make positive decisions to improve likelihood to stay healthy, prevent disease or disease complications, and maximize quality of life.

Progress to date:

Our current build includes an alpha foundation already integrated with patient registration and demographics, as well as collection of medical history as part of user signup. It has been developed as a secure and HIPPA compliant application and database. Additionally, the current platform provides secure connectivity with EMR to facilitate low burden collection of patient data for algorithms. The patient-facing interface includes a dashboard that summarizes biometric changes and predicts risk factors using well-known general algorithms that are hardcoded. While this foundation will be leveraged to speed development, it is currently a general application to drive healthy lifestyle changes. A mobile app has been completed and will be releasted in the apple app store this month. A demo is available through test flight currently. 

We have also prepared and submitted our NIH SBIR Phase I grant application for $225k (grant score due in March), designed a 40 patient usability study across two sites on both coasts (which we will complete after winning the grant), currently retain six engineers and two designers working full time on the project, assembled an advisory board of experts in the field, are applying to various incubators and regularly meeting with venture capitalists, industry experts, and investors for feedback.

Biography: Anandamoy Chatterjee has a Masters in Electrical and Computer Engineering from Cornell University. Anand has undertaken a number of high impact and dynamic machine learning and statistical algorithm projects throughout his undergraduate and graduate career. He is recipient of the highly competitive JN Tata Endowment education, awarded to only 30 students across India for his Masters in Cornell University. At Cornell, he took quantitative courses in Machine Learning for Data Science, Machine Learning for Intelligent Systems, Computer Vision, Stochastic learning and graduated within a top 1 percentile among the 2017 batch. He now works in the Data Science team at KPMG in the financial capital of the world. A great part of his daily work is to find patterns and build insights from client data by building scalable machine learning algorithms and deploying in a production environment. Biography: Diabetes is a rapidly growing epidemic across the country. As a specialist in renal dialysis nutrition, I unfortunately see some of the worst outcomes from this disease left untreated: kidney failure requiring dialysis along with diabetic neuropathy leading to blindness and tissue necrosis requiring amputations. We are at a critical moment in history in which we can make a change and reduce the rates of diabetes in this country, and what a nutritionist knows best is that prevention is key. Lifestyle- what you do day to day is what transforms people’s lives. A healthy diet and portion sizes, along with exercise and keeping up with medications and medical appointments is the key to patient success! The development of an app-based platform integrated with multiple modalities of patient data streams and the ability to use intelligent algorithms to process and present likelihood of outcomes to patients is highly innovative. These visualization tools may improve patient outcomes by driving behavioral changes in diet that can have a significant outcome on disease progression and life expectancy. I look forward to providing intellectual input on the design of the app, parameters that should be used in algorithm development, as well as general usability considerations for the intended target patient population. More specifically, much of my contribution will focus on dietary behavior, caloric consumption, prevention strategies and how those variables interact with the rest of the system and across stakeholder perspectives. The proposed technology is highly innovative and will solve a large market need, so I am happy to participate in this research project.

With the rising burden of healthcare expenditure, due to increasing prevalence of chronic diseases such as obesity, diabetes and heart disease, there is a shift towards interventions that focus on evidenced based noninvasive interventions. Over the last three years I have co-founded Integrative Health and Wellbeing program at New York Presbyterian Hospital in conjunction with Weill Cornell medicine. We think that our digital therapeutics alongside traditional therapeutics and physician observation can dramatically improve patient outcomes and clinical trial success.

We are a great fit for the Real Time Results challenge, as an artificial intelligence driven mobile health application for chronic disease prevention and care, which connects and engages all key healthcare stakeholders: patients, clinicians, employers and insurance payers. Our technology integrates several modalities of inputs including electronic health records, medical history, dietary behavior, caloric consumption, and biometric/activity monitoring and relates them to predictive algorithms. Algorithm outputs report probability of disease onset, disease progression, and life expectancy as a function of patient decision-making of clinicians in real time; while unique visualization tools, tracking, and connectivity features will positively influence patient social wellness decisions.

Social Determinants Data Challenge
Theraputix is an ideal candidate to solve the Social Determinants Data Challenge. Access to Care Challenge:
Theraputix, an inherently mobile software platform, is uniquely predisposed to solve the Access to Care challenge. We provide a new and connected service delivery model for disease prevention and care to address accessibility and cost, while maintaining effectiveness, safety and patient engagement. Patients can employ secure, encrypted platform-agnostic web and mobile apps to import existing electronic medical record and patient history into their profile. After completing a push notification or email prompted questionnaire, our software can determine eligibility for certain programs like medicaid and others as well as identifiy potential risk factors from a patients lifestyle/background via inputs including real time location. Additionally Theraputix enables patients to connect to providers from the comfort of their own home through various mediums (messaging, voice, video) which reduces the complications from "white coat anxiety", travel bottlenecks such as traffic or limited/intermittent public transportation, and the need for physical appointments which often are missed costing the system even more. Affordable Choices Challenge:
Type 2 Diabetes, a preventable chronic disease which often leads to additional complications, significantly impacts quality of life and total healthcare costs with U.S. costs in 2017 exceeding $325 billion (roughly 25% of total costs). Therefore, this disease represents a significant and costly healthcare challenge positioned remarkably well for preventative technologies like Theraputix. Through Theraputix, patients can track their biometric data using a variety of modalities of inputs and receive personalized ai driven insights from crowdsourced data libraries. These insights are also relayed to providers where they can easily export to their existing emr/ehr system and apply appropriate billing codes for care provided through the platform reducing need for additional paperwork and hospital visits. Furthermore, because we are focused on the earliest stages of chronic disease development, we can aid the care continuum in preventing additional complications like heart disease, stroke, eye problems etc. which so often present themselves after a patient has been diagnosed with diabetes or another chronic condition We know that each subsequent chronic condition increases the probability of costly emergency department visits. Our business model is focused on minimizing costs to patients and overworked primary care providers, and providing quantitative metrics of success to payers/health systems which clearly delineate affordability and impact of care.

New Orleans and Our Company

Our software can have a particularly large impact in New Orleans. Firstly, our platform is mobile first and allows patients to access care from the comfort of their own homes reducing the need to use intermittent or inefficient public transportation systems. Secondly, New Orleans exists in an area of the country called the "diabetes belt" where diabetes is about twice as prevalent, due to many social factors (increased percentage of African Americans, higher prevalence of obseity, diet norms of the Southern United States etc.). Diabetes and obesity are often a precursor to other chronic conditions such as heart disease, stroke, eye problems and more more. Patients with one one or more chronic conditions often require additional hospitalization and emergency department visit when compared to normal patients. Therefore, finally, our software has the promise of reducing hospital strain, decreasing costs to the system as a hole, and increasing patient outcomes in NOLA.

Clinical Information

This year, with the funds from our Phase I SBIR I grant, we will conduct a usbaility study of our software across two sites.  We will recruit 40 subjects including 10 healthy controls, 10 pre-diabetic subjects, 10 recently diagnosed with Type 2 diabetes, and 10 who diagnosed with diabetes for at least 5 years and have experienced severe complications. We will make every effort to include a broad range of subjects with respect to gender, age, ethnicity and race. As part of study enrollment, subjects will visit the clinic and data will be collected during the initial visit including a medical history, demographic details, existing EMR data and a historical survey on diet & caloric intake. Subjects will receive training on using the app and wearable sensor interface for diary and data
collection. Visualization reports will be presented to the subjects in the clinic for survey feedback on data presentation, understanding results, and likelihood of impacting behavior. It will be made known to the subjects that the data in these reports is fictional and not their own data. Subjects will be sent home with the data collection system and requested to use it everyday for two weeks. Subjects will be requested to input diary information and use the wearable activity monitoring every day. At the end of the two-week period subjects will complete a questionnaire to evaluate their experience with the technology. It is anticipated the questionnaire will include ten to fifteen questions using a Likert scale ranging from 1 to 5 and three questions will include a freeform text box. Questions will target their overall enthusiasm, test if they understood diary and sensors, determine usage constraints, and record stumbling blocks.

Usability results will be analyzed across all subjects to determine validation success. We expect at least 80% compliance with using the software application and wearable component at requested intervals. With respect to the questionnaires, we expect to receive average Likert scores above 3 and significant positive freeform feedback. One potential pitfall is that while users understand the system setup and use in the clinic, they forget once at home which will impact both compliance and data collection . Therefore, we will setup a 24/7 number for technical support, as well as call subjects after one week to check status. 

Regulatory Status
