About our project

The problem we solve: As the healthcare industry transitions toward a value-based care reimbursement model, there is a greater focus on accountability and the transparency of clinical quality and performance. Enhanced communication between healthcare systems and patients that provides educational and actionable content will lead to more efficient care at a lower cost. Chronic Disease management especially for people with diabetes especially with nutrition, medication adherence, lifestyle changes etc. is a very real challenge in healthcare today. In addition so is helping & keeping track of patients who have risk factors for developing diabetes with proper nutrition, exercise etc. which help to reduce these risk factors.

About our solution: Diabetes connect seeks to create a care coordination tool that incorporates virtual visits, an easy to use nutrition platform as well as remote monitoring tool which allows providers to stay connected with their patients and provide alternative touches when they are not within the four walls of their clinic. Certintell’s mobile health tool would offer integrated features to proactively improve care coordination and health outcomes while reducing the overall cost of care.

Progress to date:

We currently have the virtual visit platform, nutrition and wellness application and remote monitoring devices and would like to be able to work on creating an integrated app that would work as a diabetes monitoring platform.

Our tools for food and activity logging integrate with clinician’s existing technology and workflow, adding an advanced nutritional analysis and personalized engagement solution. Our target market is the Safety Net Market. The Total Market Opportunity for the Safety Net Market with 200K Providers and $150 per provider/month licensing fee is $360M (Agency for Health Care Research and Quality-Module 2 working with safety net practices-www.ahrq.gov) 

Projected 3 Year Growth

50K Medicaid FQHC Providers x $150 per provider per month = $90M annual recurring revenue opportunity. We expect a 20% market sharer in 3 years and therfore expect ARR of 17.5 M 

How We Will Make Money

We are a software as a service company and our revenue is based on licensing our software on a per provider per month basis. We would license our virtual visit/care coordiation tool for $150 per provider per month.

As the shift to value based care occurs our SAS models affords health centers and hospitals with an affordable solution that can help to monitor patients with diabetes and other chronic conditions and reduce the cost of health care in the short and long term.

About our Competition

While there are quite a few telehealth or virtual visit companies none of them focus specifically on the safety net population. What seperates us from the competiton is that we are the only platform that incorporates a robust virtual vist solution with EMR integration, a chronic disease management tool as well as virtual interpreting service.

We have focused our solution around the triple AIM specifically with our technology around improved patient engagement, population health and lowering costs and to our knowledge none of our competitors are providing this.

Progress with Customers to date

We have interest from channel partners and partnerships with the National Association of Community Health Centers, California Primary Care Association, Louisiana Primary Care Association and Northwest Regional Primary Care Association to name a few who are interested in helping adopt our solution in Community Health Centers and Federally Qualified Health Centers.

We currently are post revenue with our uCare Connect virtual visit platform being utilized in a number of health centers around the country.

Certintell is a social impact company and we have a partnership with the Louisiana Primary Care Association to help improve access to care for patients of community health centers in the state. How we will use the funds raised

We currently have a very robust virtual visit telehealth solution and a separate nutrition and wellness application & a remote monitoring & we would like to utilize the funding from this program to create a care coordination tool that allows providers of patients with diabetes and their care teams to be able to connect with their patients remotely as well as receive and provide real time data on nutrition, wellness & blood glucose levels.
