About our project

The problem we solve: Diabetes care is broken for many Americans. An estimated 10-12 million people living with diabetes have uncontrolled disease and are left to manage this complex condition on their own. Existing solutions in the market are too expensive, don’t fit clinical workflows or don’t address both the clinical and behavioral aspects critical to managing chronic disease. An opportunity exists to use data, combined with human expertise to deliver truly comprehensive care in way that serves patients and fits how primary care is delivered today.

About our solution: The CDMP solution includes both cloud-based and mobile tools for data collection in between visits, and a single-screen patient dashboard, accessible directly from within their EHR. The primary care team works collaboratively with the patient to gather data on educational and nutritional care plans, home monitor information, surveys, and other activities. This patient data is then processed with clinical data from the EHR and presented together with key behavioral information to provide a comprehensive view of the patient. CDMP is available as an integrated technology platform or a full turnkey service for primary care.

Bio: Executive with 25+ years experience in health care (digital health and life sciences). In industry, have had success in sales, financing, licensing and business development. Led or participated in raising over $60 million venture capital and completed business development deals that brought in nearly $10 million cash proceeds. Roles have included CEO, CFO and head of business development. He has a strong personal network and history of volunteer activities within health care. He co-founded and co-heads a 1350-member non-profit organization (Philadelphia Health IT Circle) dedicated to healthcare innovation. Mr. Heifetz was also on the board of HIMSS North America and previously served on the board of the Wharton Health Care Alumni Association.

Biography: Boz is building on a long career in health care and pharmaceuticals to develop Saturn Care’s sales and market presence. A CPA by training, the majority of his experience has been with the Sanofi organization. This included management positions in both the U.S. and France. Most recently, Boz served as CFO for Locemia Solutions, a developer of a novel hypoglycemia rescue device that exited via a sale to Eli Lilly.

Clinical Information

CDMP is the result of over 12 years and $50 million of grant-based research prior to the formation of Saturn Care. During that period, two key published randomized controlled studies were completed in partnership with a consortium including Joslin Diabetes Center in Boston, MA and Baystate Health in Springfield, MA.

 

The purpose of the first study was to evaluate the clinical usefulness of a nurse-led diabetes care program (Comprehensive Diabetes Management Program, CDMP) for poorly controlled Hispanic type 2 diabetes (T2DM) patients in an urban community health center setting.

The study enrolled 21 patients in the intervention arm plus 18 patients into an attention control arm, and was conducted in an urban Latino community health center. Among the findings of the 12-month study were:

  • the intervention group experienced a 1.6% drop in HbA1c, which is greater than what is seen with many drug therapies
  • Nearly 2x higher proportion of patients met clinical goals for A1c, blood pressure, eye exams and foot exams
  • Greater improvements in Diabetes Distress and treatment satisfaction were seen

The overall conclusion was, ”The CDMP intervention was more effective than an attention control condition in helping patients meet evidence-based guidelines for diabetes care.”

Results were published in the Diabetes Educator, which is the scientific journal of the American Association of Diabetes Educators. (https://www.ncbi.nlm.nih.gov/pubmed/21918206)

A larger follow-on study was completed under a RO1 grant from the National Institutes of Health. That study had a similar protocol, but enrolled 399 poorly controlled patients and included 5 visits over 6 months. Again, the population included a low income, poorly controlled, Latino population. In this case, the control arm (“usual care”) received a Robert Wood Johnson Foundation funded diabetes self management education program.

Highlights from the study included:

  • Twice as many intervention patients reached an A1c goal of <7.0 vs. the control arm
  • The intervention group saw nearly a 0.8% drop in A1c relative to the control arm
  • Significantly lower diabetes distress at follow-up for the intervention group vs control

In conclusion, “The diabetes dashboard (ie, CDMP) intervention significantly improved diabetes-related outcomes among Latinos with poorly controlled T2D compared with a similar diabetes team condition without access to the diabetes dashboard.”

The study was published in the April 2015 edition of Diabetes Care, which is the clinical journal of the American Diabetes Assocation.

Regulatory Status

At this time, we do not require FDA clearance to market the current version of CDMP. We have an alpha version of a clinical decision support tool in development, and we are awaiting the upcoming FDA guidance on CDS before determining a strategy for release.

    Pottstown, PA-based PMSI has selected the CDMP platform for use in their Chronic Care Management (CCM) program. The CCM program is a comprehensive support service of the patient education team under the leadership of Debbie Zlomek, RN, CDE, and is open to all Part B Medicare patients living with diabetes and one or more other chronic conditions.
